When you make your appointment, you will receive an emailed link to allow you to log in to our Therapy Appointment patient portal where you will be able to complete all required forms, enter insurance information and a payment method. This information is required prior to the first appointment. We find that first session is a more comfortable and more efficient one for you and your therapist if the information is in hand. FOR MINORS: a parental consent form is required to be signed by both parents.
